Volunteer Waiver and Release

TERMS AND CONSENT

• My services to LFFAC are provided strictly in a voluntary capacity. I am not an employee or contractor of LFFAC.
• I will familiarize myself and comply with LFFAC’s policies and procedures applicable to volunteers and agree to abide by any terms for the authorized entry onto private property granted by private property owners for the purposes of my voluntary activities.
• I understand that LFFAC’s expects high standards of moral and ethical treatment of the animals under its care and courteous treatment of the public and other volunteers. I will adhere strictly to these standards in my capacity as a volunteer.

RELEASE

I understand that I may be handling animals or performing other volunteer activities while providing my volunteer services for LFFAC, and as a result, there exists a risk for personal injury and damage to my personal property. On behalf of myself, my heirs, personal representatives, and executor, I release, discharge, indemnify and hold harmless LFFAC, its officers, agents, servants and employees, from all claims, causes of action or demands of any nature or cause connected with my this Agreement. This includes any costs, attorney’s fees, and court costs incurred by LFFAC in connection with my volunteer services based on damages or injuries that I may incur in any way while volunteering. Such damages are not limited to but may include animal bites or scratches, accidents, injuries and personal property damage. I agree to release, discharge, indemnify and hold LFFAC harmless for any and all damage to my personal property while providing my services on a voluntary basis to LFFAC, its agents, assigns, servants and employees.

I understand that public relations are an important aspect of volunteer work at LFFAC. I therefore agree on behalf of myself, my heirs, personal representative and executors to allow LFFAC and its assigns to use any photographs, video or film taken of me for use in their public relations efforts. LFFAC will use all reasonable efforts to notify me but notification is not required for the photographs, video or film to be used by LFFAC. I further understand and agree that I am not entitled to receive any reimbursement or compensation for the use, by LFFAC or its officers, agents or assigns, of any photographs, videos or films taken of me.

I have read and fully understand and accept the terms and conditions of this volunteer agreement and shall I willingly comply with all of its conditions.
